Yes that is exactly what I mean because even though Sasuke was the one who was still alive at the end of the fight it wasn't him that ended up killing Itachi in the end it was the disease and what ended up killing Nagato was using Gedo Mazo and releasing all the tailed beasts so neither of them beat their respective opponents and they actually got kind of lucky that they lived through it even though both Naruto and Sasuke are very powerful.

I don't think it matters, I mean no, he didn't land a killing blow, but that isn't really required I guess. Basically, Itachi died because of Sasuke. Sasuke realized this, and when he found out the truth, the guilt must have activated it. Presumably, something similar happened to Kakashi, maybe when Rin died he felt responsible somehow and that did it.
After all, Itachi said you have to kill your best friend, but that's not the real requirement, it's just one way to reach the conditions. If you can kill them and not feel any remorse, they weren't really your best friend, were they? In the end, there has to be a actual cause to activate it.. Most likely an emotional one. Or put it this way, MS doesn't just magically activate just like that, you have to take something away from the experience..
